The latest version of the form handling functions is in our article wiki.
I don't think I or anybody else added checkbox and radio button stuff, but that wouldn't be too difficult.  I'll poke around a bit and let you know what I come up with.

Having a moment to rummage about I found that I added checkbox functionality in March, but probably have not tested it enough to add to the Wiki.

Radio groups will be similar, but more of a challenge as to the CSS.

Textareas are easy, I've just got to find the code I used.
